The General Medical Social Work team

The General Medical Social Work team undertook the project ‘Looking at the Acute Social Work Role in the Transition into Residential Aged Care - Closing the Gap’.

A standardised tool was developed that captured and consolidated the key social work roles in the transition into care. This tool has provided an educative role in relation to social work practice and informed other staff about the emotional impact of this process.
