Our goal is to help people reach optimum health and quality of life by restoring their independence.

Caulfield rehab

What we do

We aim to help people to return to their normal lifestyle with maximum independence and community support. We have a focus on patient centred care from the treating team and involvement of your family and significant others in the rehabilitation process.

Each treating team is comprised of medical, nursing and allied health professionals such as physiotherapy, occupational therapy, social work, speech therapy, dietetics, prosthetics and orthotics, clinical psychology and neuropsychology.

Who we care for

Rehabilitation cares for people who require rehabilitation in the following main areas:

  • Amputee
  • Orthopaedics
  • Spinal
  • Neurology
  • Musculoskeletal
  • Cardiorespiratory
  • Burns
  • Acquired Brain Injury

Once people are discharged from the rehabilitation service, community services such as Bolton Clarke, Hospital in the Home, Therapy in the Home, Community Rehabilitation and Day Centre are integrated into the person’s care plan to ensure continued support, treatment and monitoring following discharge from hospital.

There are also a range of outpatient clinics to provide long term follow up following discharge from the rehabilitation service.

How to access this service

Referral from your GP or specialist

Your GP or specialist can refer you for admission into this service by calling the Caulfield Bed Access Unit. The date of admission will depend on your care needs. The Bed Access Unit will call you or your family to arrange the day and time of admission.

What to bring

Every time you come

  • Medicare card
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Adverse drug alert card (if you have one)
  • Previous X-ray films, scans, ultrasounds or any other test results or reports
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Glasses, hearing aid(s) and/or walking frame if needed

For a clinic appointment

  • Your appointment letter
  • Any special items listed on your letter
  • TAC or WorkCover claim number (if relevant)

What to wear for rehabilitation or exercise classes

  • Comfortable day clothes, such as a tracksuit
  • Well-fitting, closed-toe styled shoes with firm grip soles. Do not wear open back, slip-on slippers

For overnight (or longer) rehabilitation stay

  • Comfortable day clothes, such as a tracksuit
  • Well-fitting, closed-toe styled shoes with firm grip soles. Do not wear open back, slip-on slippers
  • Personal hygiene items, such as shampoo, shaving cream, shaver, toothpaste and deodorant

What not to bring for an overnight (or longer) stay

  • Your handbag, wallet, purse or large amounts of cash
  • Valuables, such as jewellery or watches
