Caulfield Community Health Service (CCHS) consists of centre, community and home-based health professional services and health promotion activities. We assist people in our community to improve and maintain their health.

Promoting health in our community
Our Health Promotion Team aims to support health and wellbeing at the places our community spend their time.
This involves engaging with our diverse community to assist workplaces, sports and recreation facilities, early childhood services, schools, aged care facilities; including supported residential services and neighbourhood houses to identify, understand and explore opportunities to improve health and wellbeing.
We have adopted systems-thinking approaches to tackle population health priorities with our partners to capitalise on expert knowledge, share resources and skills to influence change on a larger scale.
Who we care for
CCHS focuses on providing services to people who live, work or study within the City of Glen Eira, City of Stonnington, City of Port Phillip and City of Bayside (North of South Road). We are also able to provide some services for people from other areas.
We are able to provide assistance for people who have a visual or hearing impairment and those requiring interpreters.

The number 605 bus stops just outside Caulfield Hospital, where we are located.
How do I make an appointment?
Once your referral has been accepted a clinician will contact you to make an appointment. Depending on the type of appointment, your first appointment may be here at the centre or in your own home.
What time should I arrive for my appointment?
Please arrive at your appointment 10 minutes early to check in. Please note that a fee payment is now required before all appointments.
Is there a fee for this service?
At Caulfield Community Health Service, we have a fair and transparent fee for service process. Fees are set in line with Government guidelines and are based on your income.
A fee for service is a financial contribution that you make towards the cost of the health care service you receive. This fee will depend upon your income level. At the time of your referral, we will ask you to declare if you are a high, medium or low income earner. We will explain to you the high, medium and low fees for the relevant services at the time of referral processing.
These income level categories and fee levels will be indexed annually. Your fee may be reassessed if your financial circumstances change or if you request a review.

How can I be involved in my care?
We encourage you, your family and carers to be involved in your care. The team will talk with you and the people who support you (with your permission) about your needs and answer any questions you have. Together we will help you achieve your aims.
What should I bring to my first appointment?
- Your appointment letter
- Relevant X-ray films, scans or any other test results or reports
- Existing prescriptions and medications
- Pension card
- Healthcare card
- Glasses, hearing aids, walking aids
- If you are attending an exercise program, you will need to wear comfortable clothing so that you can be actively involved
- Please wear comfortable shoes that are firm, supportive and enclosed
What if I need to cancel my appointment?
If your circumstances change please contact main reception on 9076 6666. We require 24 hours or more notice for cancellations or rescheduling.
What if I need an interpreter?
We can contact you via an interpreter in your language, and have an interpreter present at your appointment, at no additional charge for the interpreter.
What if I have impaired vision?
Contact us to let us know how we can help you better for your appointment.
What if I have impaired hearing?
We can contact you with either a text to your mobile phone or use the Text Telephone (TTY) with the National Relay Service. We can organise an Auslan interpreter for your appointment. There is no additional charge for the Auslan interpreter
Does CCHS need to get my consent?
Your consent to participate in CCHS programs/appointments is required at your first appointment. We will request your consent again if we need to contact anyone outside of Alfred Health (e.g. your doctor, local council).

Your information remains confidential at all times.

Can I smoke at CCHS?
Caulfield Hospital is a smoke free environment. Smoking is not permitted in any of the buildings or the hospital grounds. We also ask that you do not smoke during sessions that take place at your home.
If you would like to talk about support options for quitting smoking, please speak to your care team.
How to access this service
Caulfield Access Unit CCHS - patients
Caulfield Access is responsible for intake, information and referral processing for a wide range of community and ambulatory services.
Family, carers, case managers and patients can contact Caulfield Access to access our services.
Once a referral has been received, a Care Coordinator will phone the patient to discuss their needs and organise appropriate services. The Care Coordinator will provide the patient with their phone number. Patients are encouraged to contact the Care Coordinator if they have any concerns.

If you are 65 years or over (50 years or over for Aboriginal or Torres Strait Islander people), make a referral via My Aged Care.
Compensable clients
Please note, we do not see clients who have access to TAC, DVA, Workcover, and private health. These clients should seek services through these programs.
What to bring
Every time you come
- Health Care Card and/or concession card (if you have one)
- Adverse drug alert card (if you have one)
- Previous x-ray films, scans, ultrasounds or any other test results or reports
- Medicines you need to take while you are here
- List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
- Glasses, hearing aid, walking frame.
For a clinic appointment
- Your appointment letter
- Any special items listed on your letter.
What to wear for rehabilitation or exercise classes
- Comfortable day clothes, such as a tracksuit
- Well-fitting, closed-toe styled shoes with firm grip soles. Do not wear open back, slip-on slippers
