Aged Care specialises in care for people with chronic or complex conditions associated ageing, cognitive problems, chronic illness or disability.

Clinical service overview
In addition to our Geriatric Evaluation and Management (GEM) inpatient service, we have a specialised Delirium and Dementia Unit and a GEM at Home service.
We aim to work in partnership with patients and their families to improve independence, and/or reduce their long-term health needs or the need for community support. We provide a comprehensive assessment, treatment and management service.
Staff within the service work together as part of a interdisciplinary team and are highly skilled health professionals. Patients will be able to access nursing, medical and support staff 24 hours a day. Allied Health support is generally available Monday through Friday.

Call Caulfield Bed Access
We accept referrals for direct admission into this service from GP's and specialists. You need to call the Caulfield Bed Access Unit to arrange admission. You may also need to complete an Admission Request Form. Admissions are prioritised depending on patient needs and the patient and/or family is called regarding admission time and day.
