Acquired Brain Injury Community Rehabilitation

What we do

We are part of the ABI Rehabilitation Centre.  We provide a specialist service to people with a severe brain injury and their carers from the Victorian community. In addition we provide allied health, nursing and medical staff support and advice to clinicians working with people who have an ABI.

ABI Community Rehabilitation offers:

• a home-based service to address treatment goals that can only be met in the person's home environment
• a community-based service to address treatment goals that can only be met in the community
• a centre-based service that includes group and individual based treatment dependent on the need of the person
• other services such as telephone, Skype and Telehealth where appropriate

Who we care for

Acquired Brain Injury Community Rehabilitation supports people with a severe brain injury following discharge from hospital and who require further rehabilitation. This ensures they will be able to continue with specialist rehabilitation as an outpatient. Ongoing rehabilitation will enable people to reach their full functional potential as well as support re-integration into the community.

The service can be accessed by people with a severe brain injury who are unable to access other community rehabilitation services. This includes people who are residing in residential facilities and special accommodation.

Once a referral has been received, either the manager or a senior member of the Community Service will be in contact to discuss the referral.

Once accepted, the client or significant other will be contacted to discuss their needs and an appropriate assessment date will be organised.

What to bring

Every time you come

• Medicare card
• Health Care Card and/or concession card (if you have one)
• Private health insurance card (if applicable/if you want to use it)
• Adverse drug alert card (if you have one)
• Previous X-ray films, scans, ultrasounds or any other test results or reports
• Medicines you need to take while you are here
• List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
• Glasses, hearing aid(s) and/or walking frame if needed

What to wear for rehabilitation or exercise classes

• Comfortable day clothes, such as a tracksuit
• Well-fitting, closed-toe styled shoes with firm grip soles. Do not wear open back, slip-on slippers
