How do I provide feedback about my care?
We welcome and encourage patients and their families to give us feedback about their experiences. Your feedback is important to us and helps us know what we are doing well and where we need to improve. You can provide feedback in different ways.
Make a suggestion to improve care
- Talk to the staff caring for you
- Complete a feedback form online. Some of our services have paper ones. Ask staff to provide you with a copy or get one from main reception areas
- Contact our Patient Liaison Office.
Compliment our staff
- Talk to your healthcare team or the manager of the area
- Complete a feedback form online. Some of our services have paper ones. Ask staff to provide you with a copy or get one from main reception areas
- Leave a written message or card for the staff.
Make a complaint
- If you or your loved one is a current inpatient, talk to the staff caring for you/them or the manager of the area to tell them your concerns
- Complete a feedback form online (select "Make a complaint" from the Feedback type dropdown list)
- Contact the Patient Liaison Office by phone on (03) 9076 8001 should you require more information and support.
Due to the Coronavirus (COVID-19) the Patient Liaison Office is operating on remote staffing arrangements but we do remain open.
We appreciate your patience in these uncertain times. To limit delays in returning telephone calls, we ask that you continue to contact us via email at feedback@alfred.org.au or using the feedback form.
We will endeavour to acknowledge all feedback within 48 business hours, however due to an increase in feedback it may take us longer to get back to you.
Provide a detailed account of your experience
- Complete the Patient Experience Survey with trained volunteers or staff while you are in hospital. Alternatively, you can complete the survey online after you leave.
- The Patient Experience Survey is a series of questions that takes 10 minutes to complete. The information is used by us to find out what we are doing well and where we need to improve.
Provide feedback in another language
- Request an interpreter and speak to your healthcare team about your feedback
- Call 13 14 50 and ask to speak to the Patient Liaison Office with an interpreter
- Write to us in your own language
- Request to complete the Patient Experience Survey in Greek, Russian, Italian, Simplified Chinese or Turkish.
